Student Support Services Transition Program
Mission Statement:
The mission of the Student Support Services transition program is to assist students in coordinating and managing activities that will help them adjust to employment and/or post secondary education after leaving high school. The transition process begins when a child moves from pre-school to kindergarten and continues through elementary to middle school, middle school to high school and high school into the adult world. Transition involves the careful coordination of individual needs matched to available programs with the collaboration of parents, teachers, the student and other relevant individuals in the community. An emphasis is placed on high school students with disabilities who are assisted in developing a transition plan to connect their high school activities to their goals for an independent life after completing high school. Overall, the transition program provides support for parents and staff invested in helping students move through the various stages of their educational experience up to and including the move to college, technical schools, and/or post high school employment.
